Parental Consent:
A parent or guardian must sign a consent form before their student can access Denver Health Pediatric services. Once this form is submitted, students can use the clinics for the entire school year. Open communication between students, parents, and the clinic is encouraged to ensure that health care decisions are made together.
To sign the consent form, visit the Denver Health website or use the QR code provided. Hard copies are also available at the clinic.
Under Colorado State Law, students can confidentially access certain services, including sexual health and behavioral health services. All other services require parental or guardian consent.
Cost & Insurance:
There are no fees for the services provided at the clinic. Insurance may be billed, but families will not have co-pays, deductibles, or fees to worry about. Community Health Care Advisors are available to assist with insurance inquiries and enroll families in coverage if needed.
